CHIEF COMPLAINT: 38 year old female, home maker came to hospital with chief complaints of  Poor stream of urine since 1 month, Dysuria since 15 days,Burning micturition since 10 days,Pain in lower abdomen since 10 days. HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic 1 month back , then she developed poor stream of urine. she developed pain during micturition since 15 days which is insidious in onset.  Burning micturition since 10 days.  Lower abdominal pain since 10 days which is of

I have been given this case in order to solve in an attempt to understand the topic of patient's clinical data analysis to develop my competency in reading and comprehending clinical data and come up with a diagnosis and treatment plan.  A 55 year old male patient who is shepherd by occupation  came to the hospital with the chief complaints of burns over the face due to oil spillage and pain in the abdomen since three days. HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic  3years back ,then he had fever  associated with body aches for which he went to a hospital where he was diagnosed with diabetes mellitus and was on oral hypoglycemics since then.patient had intake of 90ml of alcohol in the morning and came to home , his daughter was preparing the sweet

History of presenting illness  The patient was asymptomatic 7 days back when he developed fever which was insidious in onset intermittent , low grade and subsided temporarly on taking medication . it was associated with headache (in the frontal region), neck pains , back pain , body pains .  Past history  No history of diabetes, hypertension ,asthma, TB ,epilepsy .

A 73 years old female patient who is  an agricultural worker by profession from peddasuraru, came to casualty with c/o diminision of vision since 2 years.   History of present illness: Patient was apparently asymptomatic, 2 years back when she developed diminision of vision in both eyes which is insidious in onset, gradually progressive.  History of past illness: Right eye Got operated with SICS  PCIOL K/c/o -HTN,asthama No H/o - TB,DM,CAD Personal history:  Diet : mixed  Appetite : adequate   Bowel movements : regular  Bladder movements : regular  Sleep : distrubed  Addictions : occasionally toddy.  Allergies : no allergies General examination : Pallor : absent  Icterus : absent Cyanosis : absent Clubbing : absent Lymphadenopathy : absent Pedal oedema: absent                              Left eye                                  Right eye Vitals: Temperature : 97.5°F BP : 130/70 mmHg PR : 85 bpm RR : 14 com SpO2 : 98% Ophthalmic examinations:
